الجلوكوما مجموعة أمراض قد تؤذي العصب البصري، وقد تتطور بعض أنواعها دون أعراض واضحة. الخطة تعتمد على نوع الجلوكوما ودرجة الضرر وضغط العين وعوامل الخطر.
Glaucoma is a group of diseases that can damage the optic nerve. Some forms progress with few symptoms, so management depends on glaucoma type, existing damage, intraocular pressure and individual risk.

لا يعتمد التشخيص على قياس ضغط العين وحده. قد يتضمن التقييم فحص العصب البصري، قياس الضغط، تقييم زاوية العين وسمك القرنية، واختبارات المجال البصري أو تصوير العصب عندما تكون متاحة ومطلوبة.
Diagnosis is not based on eye pressure alone. Assessment may include optic-nerve examination, pressure measurement, angle and corneal evaluation, visual-field testing and optic-nerve imaging when indicated and available.
أهداف العلاج
Treatment goals
الهدف هو إبطاء أو منع مزيد من الضرر قدر الإمكان والمحافظة على الوظيفة البصرية المتبقية. مستوى الضغط المستهدف والخطة يختلفان من مريض لآخر.
The goal is to slow or prevent further damage as much as possible and preserve remaining visual function. Target pressure and treatment strategy vary between patients.
القطرات والعلاجات الأخرى
Drops and other treatments
قد يشمل العلاج قطرات للعين، وقد تكون الخيارات الليزرية أو الجراحية مناسبة في بعض الحالات بحسب نوع الجلوكوما ودرجة المرض والاستجابة للعلاج. إذا كان العلاج المطلوب يحتاج جهازاً أو منشأة غير متوفرة داخل أوسارا، يتم ترتيب الإحالة أو الإجراء في منشأة مناسبة ضمن خطة الرعاية.
Treatment may include eye drops, while laser or surgical options may be appropriate in selected cases depending on glaucoma type, disease severity and response to treatment. When a required treatment needs equipment or an operating facility not available at OSara, it is arranged through an appropriate facility as part of the care plan.

إذا أصبحت الجراحة جزءاً من الخطة، تُناقش الحاجة إليها ونوع الإجراء ومكانه مع المريض. العمليات التي تحتاج غرفة عمليات تُجرى في مستشفى أو منشأة جراحية مناسبة، مع بقاء الاستشارة والتخطيط والمتابعة ضمن مسار الرعاية مع د. سارة.
If surgery becomes part of the plan, the need for it, the procedure and its location are discussed with the patient. Procedures requiring an operating theatre are performed at an appropriate hospital or surgical facility, while consultation, planning and follow-up remain within the care pathway with Dr. Sara.
المتابعة طويلة الأمد
Long-term follow-up
الجلوكوما غالباً تحتاج متابعة دورية لأن الضغط وحده لا يصف كل شيء. تُعدّل الخطة حسب التغير في العصب أو المجال البصري والتحمل والالتزام.
Glaucoma often requires ongoing follow-up because pressure alone does not tell the whole story. Management is adjusted according to optic-nerve or visual-field change, tolerability and adherence.
