Punch minigrafting هو أحد أساليب جراحة البهاق، ويعتمد على نقل طعوم جلدية صغيرة جداً من جلد طبيعي اللون إلى بقع مختارة ومستقرة بعد تقييم دقيق للحالة.
Punch minigrafting is a vitiligo surgery technique that transfers very small grafts of normally pigmented skin into selected stable vitiligo patches after careful assessment.

The procedure is considered in selected cases after assessing stability, lesion size and location, previous treatment and suitability of donor and recipient skin.
كيف يتم الإجراء؟
How the procedure works
تُؤخذ طعوم صغيرة جداً بأداة punch من منطقة جلد سليمة، وتُجهز نقاط استقبال مقابلة داخل البقعة ثم توضع الطعوم وتُثبت وفق الخطة الجراحية.
Tiny grafts are harvested with a punch from normally pigmented skin. Matching recipient sites are prepared within the vitiligo patch, and the grafts are placed and secured according to the surgical plan.

العناية بالضمادات وتقليل الاحتكاك أو الحركة في بعض المواضع قد يكون مهماً في الفترة الأولى. تختلف مدة العودة للنشاط حسب موضع وعدد الطعوم وطبيعة العمل.
Dressings and protection from friction or movement may be important early in recovery. Return to normal activity varies with the treated site, number of grafts and the patient's routine.

How pigmentation develops
بعد استقرار الطعوم في المنطقة المستقبلة قد تبدأ الصبغة بالانتشار تدريجياً من مواقعها إلى الجلد المحيط. درجة الانتشار واللون النهائي تختلف بين المناطق والمرضى.
After the grafts have settled in the recipient area, pigmentation may gradually spread from graft sites into surrounding depigmented skin. The degree and colour match vary by site and patient.

Advantages and limitations
يمكن أن يكون Punch Minigrafting مناسباً لبعض البقع الصغيرة أو المحددة، بينما قد يكون أقل عملية للمساحات الكبيرة أو بعض المواقع. ومن الأمور التي تُناقش قبل الإجراء احتمال اختلاف اللون أو الملمس، بروز بعض الطعوم أو ظهور مظهر مرصوف (cobblestoning)، إضافة إلى آثار بسيطة في المنطقة المتبرعة أو المستقبلة.
Punch minigrafting may suit selected small or localized patches but can be less practical for larger areas or certain anatomical sites. Potential limitations discussed before treatment include differences in colour or texture, raised grafts or a cobblestone appearance, and small donor- or recipient-site marks.

Comparison with NCMT
الـ punch grafting ينقل وحدات جلدية صغيرة، بينما NCMT ينقل معلقاً خلوياً إلى مساحة محضّرة. الاختيار يعتمد على الحالة وليس على اسم التقنية وحده.
Punch grafting transfers tiny skin units, while NCMT applies a cell suspension over a prepared area. Choice depends on clinical context rather than the procedure name alone.
